Bowel clear out, lactulose, enemas
Antibiotics
Supportive (ITU, airway support, NG tube)
What are the signs of spontaneous bacterial peritonitis?
Abdominal pain Fever, rigors Renal impairment Muscle wasting Signs of sepsis, tachycardia, temperature
What are the treatments for spontaneous bacterial peritonitis?
IV antibiotics
Ascitic fluid drainage
IV albumin infusion
What is the presentation of alcoholic hepatitis?
Jaundice
Encephalopathy
Infection common
Decompensated hepatic function (low albumin and raised prothrombin time/INR)
What are the signs of alcohol hepatitis that are used for diagnosis?
Raised bilirubin
Raised GGT and AlkP
Alcohol history
Excluded other causes
What is the treatment for alcoholic hepatitis?
Supportive Treat infection Treat encephalopathy Treat alcohol withdrawal Protect against GI bleeding Airway protection/ITU care Steroids (ONLY if severe) Nutritional = thiamine, frequent feeds, high energy
What is the prognosis for alcoholic hepatitis?
Depends on abstinence/ongoing alcohol consumption
Decompensating liver disease = 70% mortality in 5 years
Encephalopathy = 64% in 1 year
What is steatohepatitis?
Fatty liver + inflammation
non-alcohol but similar to alcohol induced damage
What are the signs of steatohepatitis?
Asymptomatic
Raised alanine amino transferase
Fatty liver on USS
Liver biopsy
What is the treatment for steatohepatitis?
Weight loss, exercise
