What is AKI stage 1?
Greater than or equal to 1.5 times the baseline creatinine or UO less than normal for more than 6 hours
Stage 2 AKI
Greater than or equal to two times the baseline creatinine or UO less than normal for more than twelve hours
Stage 3 AKI
Greater than 3 times the baseline or UO <0.3 for 24 hours or anuria for 12 hours
How do you obtain a baseline creatinine
Lowest value within 7 days of current value (if unavailable go up to one year)
If a patient has an AKI and a negative urinalysis what is the cause
Prerenal
When do you admit for an AKI
Stage 3 or infected obstructed system
Untreated prerenal AKI can lead to
Acute tubular necrosis
Treat prerenal
Fluid challenge
If >1000mls and no improvement seek help
What to do in renal AKI
Urinalysis and consider renal biopsy
When give dialysis in AKI
Persistent anuria and uraemia
If you see hyperkalaemia in AKI what do you do
ECG
Treat hyperkalaemia
10mls 10% calcium gluconate
Insulin with 50mls 50% dextrose
Salbutamol Nebs
What are the life threatening complications of AKI
Hyperkalaemia Fluid overload Severe acidosis Uraemic pericardial effusion Severe uraemia

NSAIDS

Diuretics
What drugs cause renal AKI
Gentamicin
