Acromegaly
A
a hormone disorder in adults where there is excess growth hormone (somatotropin)
• continued growth of extremities -> large face, hands and feet

Impacts of growth hormone - DIRECT EFFECT
A
- Liver releases more glucose into the blood
- Body retains more nitrogen in muscles leading to muscle growth
- Osteoblasts are stimulated causing bones to thicken
- Daibetogenic effect - increase in blood insulin levels like diabetes

Impacts of growth hormone - INDIRECT EFFECT
A
- GH stimulates certain tissue like liver, muscles, bones, kidney
- Produce somatomedin C (insulin like growth factor 1)
• Promotes cellular metabolism
• Prevents cell death
• Helps cells divide and differentiate through out the body

Causes of Acromegaly
A
1. pituitary adenoma • Benign tumor of somatotrophs in the anterior pituitary -> makes excess GH -> excess insulin like growth factor 1 2. Hypothalmic tumors -> excess GHRH released 3. Ectopic tumors elsewhere in the body 4. Multiple endocrine neoplasia Type 1 ( Wermer Syndrome)

Symptoms of acromegaly
A
May take years to be recognised
- Growth of bones, hands, feet and lower jaw
- protrusion of the forehead
- Soft tissue swelling in hands,feet, face and tongue
- Increase size of organs
- Excess swelling

Assoc conditions risk factors
A
- Carpal Tunnel syndrome - increased pressure on nerves from muscle growth
- diabetes mellitus - diabetogenic effects from GH
- Congestive heart failure - enlarged heart
- Gastrointestinal cancer - colon polyps

Diagnosis
A
- Elevated levels of insulin like growth factor 1 and growth hormone
- MRI - confirms pituitary tumor

Treatment
A
- Surgery - pituitary adenoma removed
2. Medication - somatostatin anologs / Growth hormone antogaonist to stop GH binding to target tissues
