What is Bishop’s score used to access?
Clinically accesses the cervix, the higher the score the more success an induction will be.
Looks at: dilation Length of cervix (effacement) postion Consistency of cervix Station
What can be given to help stimulate adaquate contractions?
IV oxytocin
Want 4-5 contractions in 10 minutes
What are indications for induction?
Diabtetes
Post dates Term + 7 days (as increasing risk of still birth)
Maternal need for planned delivery (needs treatment e.g. for DVT)
Fetal reasons (growth concerns or oligohydramnios)
Maternal request
What possible intrapartum complications can occur?
REMEMBER P’s
Power- inadequate uterine activity
Passages- Cephalopelvic disproportion (CDP) or obstruction (e.g. fibroids)
Passenger- Malposition and Malpresentation
What is CPD?
Cephalopelvic disproportion - mismatche between size of babies head and mothers pelvis
Describe the difference between Malposition and Malpresentation
Malpostion - not in occipito-anterior position
Malpresentation- not in cephalic lie.
What is a 3rd degree tear?
Involves anal sphincter
What is a 4th degree tear?
Involves rectal mucosa
What is placenta previa?
When the placenta is attached too low in the uterus (with 10cm of opening or less)
