Physiologic Criteria for Trauma Center Transport
GCS less than or equal to 13
SBP<90
Resp. rate<10 or >29, or need for ventilatory support
Geriatric SBP<100
Infant resp. rate<20
Anatomic Criteria for Trauma Center Transport
Open or depressed skull fracture
Penetration of head/neck/torso/extremities proximal to elbow/knee
Chest wall instability/deformity
Abdominal tenderness
Suspected pelvic fracture
New onset paralysis
Two or more proximal long bone fractures
Crushed/degloved/mangled/pulseless extremity
Trauma with Burns
Amputation proximal to wrist or ankle
Mechanism Criteria for Trauma Center Transport
Fall, adult 15 feet or greater
Fall, peds greater than 10 feet or 3 times height
Auto vs. ped or bicycle greater than 20 mph
Motorcycle crash greater than 20 mph
Ejection from vehicle
Death in same vehicle
Intrusion including roof greater than 12” at occupant site
Intrusion including roof greater that 18” at any site
Criteria for TBH contact
Geriatric> 65 Y/O
Pediatric
Anti-coagulant/anit-platelet therapy
Pregnancy greater than 20 weeks
MVC greater than 40mph
Loss of consciousness reported
EMS provider judgement
Desitnation Decision
Adult pt identified as critical trauma to closest trauma center
Peds pt identified as critical truama to closest peds trauma center if<30mins if not closest trauma
If transport is>30mins consider helicopter
Critical Trauma Criteria
If airway is compromised and cannot be managed, transport to closest
Traumatic full-arrest
Burns
