AMS Pale, ashen, cyanotic, cool, wet skins Altered respirations Tachycardia Hypotension Arrest
Pertinent Differential Diagnoses
Cardiogenic shock (arrhythmia, infarct) Distributive shock (anaphylaxis, neurogenic, sepsis) Hypovolemic shock (hemorrhagic shock, blood loss, burns) Obstructive shock (embolism, tamponade, tension pneumo)
Emergency Stabilization A
Do not delay transport with non-essential treatment, scene time less than 10 minutes
Control bleeding- direct pressure, tourniquet
Position pt- no Trendelenburg
Warm pt
Make hospital contact
Start large bore bilateral IVs during transport
250 NS Bolus
Emergency Stabilization B
IO access if necessary
Unilateral Needle T for S/S of tension pneumo with rapidly progressing respiratory distress unrelieved by less invasive means
Bilateral Needle T for traumatic arrest with suspected torso trauma
Disposition
Blunt trauma arrest goes to closest facility
Penetrating trauma arrest goes to closest trauma center if no more then 10 mins farther than closest, otherwise go to closest
