What is Horner’s syndrome?
A
An interruption of the sympathetic nerve supply to the eye.

Causes
A
- Lesion of the primary neuron
- Brainstem stroke
- Tumor of the preganglionic neuron
- Trauma to the brachial plexus
- Tumors (e.g., Pancoast)
- Infection of the lung apex
- Lesion of the postganglionic neuron
- Dissecting carotid aneurysm
- Carotid artery ischemia
- Migraine
- Middle cranial fossa neoplasm

Symptoms
A
Symptoms depends on affected neurons.
Miosis, partial ptosis, anhidrosis, ophthalmoplegia, nystagmus, iris heterochromia
Postgang. => no dilation

Diagnosis
A
Clinical diagnosis
Cocaine drop test, paradrine test, hydroxyamphetamine eye drops helps to distinguish preganglionic from postganglionic, dilation lag test
(CT/MRI)

Treatment
A
Depends of underlying. Neurosurgery might be needed for aneurysm-related.
