37: Opioids Flashcards
How long do IR/ ER/ epidural / suppository opioids typically last?
- IR: 3-5hrs
- ER: 8-24 hrs
- Epidural: up to 24hrs
- suppository: 3-7 hrs
AEs with chronic opioid use
physical dependence, constipation, low libido, cross tolerance, loss of appetite, brain damage, tolerance, hepatic damage
sedatives + hypnotics AEs
increased CNS depression and resp depression
opioids + anti-psychotic AEs
increased sedation, possible CV effects, variable resp depression
opioids + MAOI’s
relative contraindication bc high incidence of hyperpyrexic coma and HTN
three opioid receptors
mu, delta, kappa
mu receptor 4 functions
- spinal analgesia
- resp inhibition
- slow GI transit
- modulates H and nt release
delta receptor 2 functions
- spinal analgesia
2. hormone and nt release
kappa receptor 3 functions
- spinal analgesia
- slowed GI transit
- psychomimetic effects
three endogenous opioid peptides
- endorphins
- enkephalins
- dynorphins
what endogenous opioid peptide works best on the three opioid receptors
- mu: endorphins
- delta: enkephalins
- kappa: dynorphins
morphine works on which receptors
all three (mu, kappa, delta)
methadone works on which receptors
mu
buprenorphine works on which receptors
all three
naloxone works on which receptors
mu, delta
