When are the two times as a desk officer you notify the sick desk supervisor in regards to a UMOS being sick
After you do call back to where the member is staying while sick and they are not at the location
And
If special or unusual circumstances exist regarding member reporting sick (out of state/ country)
If UMOS is unable to visit department surgeon who do they notify?
Notify CO and sick desk supervisor.
What’s the procedure for reporting sick outside city and residence counties?
- Phone med division sick desk or send telegram to CO medical division.
- give following information
A) rank, name, shield, command
B) present location and number
C) nature of disability
D) name address and number of attending doc
E) date of probable return to duty
F) scheduled app in court, trial room, etc - obtain letter from doc and send to CO med div
- comply with instructions from CO med dog and visit med div or district surgeon as required.
Who do u have to notify for department ambulance?
Telephone emergency medical squad, emergency service unit
No department ambulance transport for
EDP’s or airborne contagious diseases.
Who do you notify if you suspect member is malingering?
Notify medical divisions absence control
What are the steps when you as an UMOS is injured in the performance of police duty?
1) request patrol supervisor to respond
2) notify desk officer of circumstances of injury, name and address of witnesses.
3) request witnesses to await arrival of supervisor.
If platoon commander is not available to complete A and B of the line of duty injury report, the patrol supervisor will complete
part B and the desk officer will designate a second supervisor assigned to the precinct of occurrence to complete part A. If second supervisor not available, The desk officer will complete A if no other supervisors will complete
Who does desk officer, precinct of occurrence notify if member is not assigned to precinct of occurrence?
Notify injured member’s command.
If member dies or is likely to die, who do you notify?
Precinct commander or duty captain.

The desk officer will notify the sick desk supervisor when any of the following conditions relating to line of duty injury exist:
- Any request for LOD by an off duty UMOS

- The desk officer will always obtain a sick control number whether injured member reports sick or not.

- photocopy LOD report and forward to medical division, attn: LOD desk
- immediately deliver LOD report and witness statement to Precinct commander/ duty captain

Upon learning about the death of an active (uniform or civilian) member of the command or retired (uniformed or retired) member of the command that resides within the command:
Desk officer request information
Rank, name, time of death, funeral time, cemetery, etc.
if UMOS whether pall bearers or escort will be required. If active member whether death was caused as a result of performing police duty or otherwise.
He then telephones the communication section with the information.
If member is an active MOS, notify IAB.
Notify unit commander.
Post up info on bulletin board if active member.
The trauma counseling team will respond to all incidents involving:
- MOS is shot, killed, or seriously injured in the performance of duty
- any incident pursuant to a request from a. Captain or above
Who requests trauma counseling team?
The desk officer calls operations unit for the trauma counseling team.
The operations unit supervisor will then notify sick desk supervisor, provide detailed information and request response of trauma team.
When is the followup meeting with MOS after initial meeting with trauma counseling team?
48 hrs after event or initial contact.
Any additional meetings will be scheduled or to six weeks after the incident.
Which form does the ICO retain for the duration of the members physical therapy treatment?
Retain the notification of line of duty therapy authorization and photocopies of certification of line of duty/authorization for treatment and authorization for release of health information pursuant to HIPAA.
Who does the ICO a return copy of completed therapy verification form to?
Medical bills department of the medical division.
District surgeons may authorize 12 physical therapy sessions and repeat authorization two additional times for an additional of 36 sessions. Who does the MOS obtain authorization beyond 36 sessions?
Department orthopedist.
Who does MOS call go request blood when outside of NYS?
Telephone medical division during business hours or sick desk supervisor during non business hours. Provide pedigree and additional info.
What is the MOS’s steps to apply for transfer?
- Prepare application for transfer and it must be typed.
- obtain supervisor/witness signature on form.
- submit all copies to CO.
- once you get back white copy, forward to CO, Personnel orders division in a sealed envelope.
- to withdraw app, prepare 4 copies of typed letterhead to CO, Personnel orders division to withdraw.
What are the minimum requirements to request a career program transfer?
3 years of service and 12 career program points.
After CO returns the completed application for PO/Det career program the MOS forwards where?
- Original to members outreach section in a sealed envelope.
- copy to personnel officer - current borough/bureau.
Application is expunged after two years from date of submission. Must submit again if interested.
Cannot utilize the program for two years from date of last career transfer.
What is the maximum comp time a UMOS may accumulate?
5 days and it must be used within one year.
If a member is AWOL for 2 hours at the beginning of tour
Desk officer makes telephone inquiries to:
- Command roll call
- Sick desk supervisor
- Court sign in rooms
- Member’s residence
Check the following department records
- telephone record
- roll call change sheets
Make inquiries with command personnel.
Request DO in member’s resident precinct to have supervisor respond to the member’s residence.
*if outside city request inter-city correspondence unit to make notification to LE concerned to respond to member’s residence.
