What does the prescription in a patient’s chart include?
The prescription includes the type of radiation, the beam energy, the dose per fraction, the total dose, the amount of fractions, bolus, gating, imaging, etc.
Examples: photons or electrons; Examples: 6 MV, 15 MV, GE 9E, etc.
How are linear accelerators and Cobalt-60 machines operated?
Linear accelerators are on for a specific amount of monitor units. Cobalt-60 machines are on for a specific amount of time (minutes).
Gamma Knife and HDR units also use time.
What should be documented in the patient’s chart regarding treatment?
The amount of time or monitor units for each patient’s treatment should be documented in the patient’s chart or treatment plan.
How are daily and accumulated doses recorded?
Therapists record the daily dose and accumulated dose in the patient’s chart alongside the date of the treatment and fraction number.
Typically, there are two different columns in the treatment chart.
What is an example of a curative dose?
An example of a curative dose is daily fractions of 180 to 200 cGy to a total dose of 5,400 to 6,000 cGy.
What is an example of a palliative dose?
An example of a palliative dose is a daily fraction of 300 cGy to a total dose of 3,000 cGy.
What does B.I.D. treatment mean?
B.I.D. treatments mean the patient is treated twice in one day, with at least six hours passing before the second treatment is given.
How are fraction numbers and elapsed days recorded?
Each fraction number should be recorded along with the date and dose. Each day from the start of treatment to the end of treatment is recorded as ‘elapsed days’.
Some facilities begin recording the first treatment day as ‘0’, while others begin with ‘1’.
What information is included in the treatment plan?
The treatment plan includes information on the type of radiation and energy, with each field having its own name and field number.
Example: 1 AP.
What is a Dose Volume Histogram (DVH)?
Doses to other organs may be found on the DVH (dose volume histogram).
What should patient setup instructions include?
Information from the simulation should be recorded to reproduce the setup for the patient’s treatment.
Examples of necessary information: gantry angle, SSD, collimator size, collimator angle, couch angle, MLCs, patient’s position, neck rolls, immobilization devices, tattoo locations.
