What is informed consent?
Informed consent forms must be signed by the patient and the doctor.
A parent, power of attorney, or health care proxy can sign in place of the patient when the patient is unable to sign.
What does the informed consent form provide?
It gives the doctor permission to perform a procedure on the patient and explains the procedure, risks, benefits, and alternatives.
It is required for invasive procedures, sedation, any procedures that put the patient at risk, and procedures that use radiation.
What is written consent?
A written consent is an official agreement that has been signed on paper or digitally by everyone involved.
What is verbal consent?
Verbal consent gives the doctor permission with a verbal message.
What is simple consent?
A simple consent is expressed or implied permission given for the procedure when the patient does not have enough information about the procedure.
What is expressed consent?
Expressed consent occurs when the patient does not stop the procedure and wants the procedure to occur.
What is implied consent?
Implied consent is when it is assumed that the patient would give consent if the patient’s condition permitted.
Used in emergency situations.
What is inadequate consent?
A patient is not given enough information to make the appropriate decision.
