effect of progesterone on pregnancy
thickens cervical mucus (decreased sperm penetration). Inhibits endometrial proliferation. With estrogen, inhibts LH/FSH and thus prevent estrogen surge and thus prevent LH surge and ovulation
why do we add estrogen to OCP
suppress FSH, suppress LH surge further, stabilize endometrium, increase P receptors
second generation (nortestosterone derivatives) OCPs have what main side effect
acne and oily skin
drospirenone
analogue of spironolactone. anti-mineralocroticoid, anti-androgenic. Prescribed for acne or PMS but watch out for hyperkalemia
estrogen-related side effects
breast tenderness, nausea, depression, headaches, THROMBOTIC COMPLICATIONS
Contraindications for OCPs
smokers >35 y/o (increased CV risks), Hx of thromboembolism or stroke
what are some positive side effects of OCPs
decreased menstrual bleeding (less anemia), decrease CA risk, decreased PID (more mucus), increased bone density (consistent estrogen levels)
problem with progestin only pill
need to take EVERY DAY
most common injectable contraception
depo-medroxyprogesterone acetate (DMPA)
Mechanism of IUD
alters endometrium preventing implantation. Cu also stimulates inflammation which is spermicidal
Emergency contraception: different kinds and when effective
Plan B w/in 72 hrs. Ulipristal w/in 5 days. Cu IUD w/in 7 days
