What is the pathology in Wernicke and Korsakoff?
Thiamine deficiency
What causes Wernicke?
Usually alcoholism
Others- stomach cancer, persistent vomiting, dietary deficiency
What are the features of Wernicke?
Nystagmus Opthalmoplegia Ataxia Confusion, altered GCS Peripheral sensory neuropathy
What investigations are done for Wernicke?
Bloods
MRI
What is the management of Wernicke?
Urgent replacement of thiamine
Pabrinex
What is the complication of Wernicke?
Progression to Wernicke Korsakoff
What causes Korsakoff?
Usually alcoholism
Other- malnutrition, persistent vomiting
What are the features of Korsakoff?
Anterograde amnesia
Retrograde amnesia
Confabulation
What is the management of Korsakoff?
Urgent replacement of thiamine
Pabrinex
What is Wernicke-Korsakoff?
Acute Wernicke encephalopathy phase followed by chronic Korsakoff syndrome
