What is MG?
Autoimmune condition with antibodies against nicotinic acetylcholine receptors at NMJ
Who is MG mainly seen in?
Females <40
Males >60
What is MG associated wth?
Other autoimmune onditions
Thymoma in 15%
Thymis hyperplasia in 60%!
What is the presentation of MG?
Fatigable weakness
Early= generally eye weakness, double vision, ptosis
Becomes more severe and generalised- dysphagia, slow slurred speech etc
What investigations are done for MG?
AChR antibody
AntiMusk antibody
Single fibre EMG- fatigue
CT chest- look for thymoma
What is the 1st line management of MG?
Acetylcholinesterase inhibitors- e.g. pyridostigmine
What is the 2nd line management of MG?
Immunosuppression
IVIg
Thymectomy
What is the management of a myasthenia crisis?
Intubation/NIV
IVIg
Plasmapheresis
What is important to remember when treating patients with MG?
Lots of drugs to avoid BB Gentamycin, erythromycin, azithromycin Ciprofloxacin Lithium Opiates More
