What are the static stabilisers of the GHJ?
Glenoid Labrum Ligament Complex (coracohumeral and coracoacromial ligaments)
What is the structure of the glenoid labrum?
fibrocartilagenous pear-shaped rim which deepens the glenoid fossa
Also provides attachment for long head of biceps and triceps
What are the attachments of the coracohumeral ligament?
O: lateral border of coracoid
I: superior aspect of anatomical neck of humerus and to transverse humeral ligament
What is the role of the coracohumeral ligament?
Blends with the joint capsule and overall improves stability of the shoulder
What are the attachments of the coracoacromial ligament?
O: lateral border of coracoid
I: apex of acromion - anterior to ACJ
What is the function of the coracoacromial ligament?
Prevents superior displacement of the humerus - forms the fibroosseous arch
What are the dynamic stabilisers of the glenohumeral joint?
supraspinatus, infraspinatus, teres minor, subsacpularis, LH of biceps tendon, LH of triceps tendon
How do the rotator cuff improve stability of the shoulder?
act as contractile ligaments to keep the HoH in glenoid
What is the role of supraspinatus in GHJ stability?
prevents superior translation of humerus
What is the role of the infraspinatus and trees minor in GHJ stability?
prevent anterior translation of humerus
What is the role of subscapularis in GHJ stability?
prevents posterior translation of humerus
What is the movement of the shoulder complex to achieve full flexion?
Initially - pure GH movement to 70 degrees,
Then LR and protraction of scapular in a 2:1 ratio to achieve full 180 degrees
What is the movement of the shoulder complex to achieve full shoulder extension?
Pure GH movement to 50 degrees
Then combines with MR and retraction of scapula to achieve 70 degrees extension
What is the role of the shoulder complex in abduction?
Initial 20-30 degrees is pure GHJ
Then lateral rotation and elevation of the scapula in a 2:1 ratio to clear the acromion
Final 10 degrees there is LR of humerus to increase articulation of the HoH in glenoid
What muscles LR the scapula?
Upper traps, lower traps, serrates anterior
