What is Subacromial Pain Syndrome also known as?
Rotator cuff tendinopathy or impingement syndrome
Where does pain in subacromial pain syndrome originate from?
Supraspinatus tendon, infraspinatus tendon and the subacromial bursa
What other structures could cause pain in the same area as subacromial pain syndrome?
long head of biceps
subsacuparis
supraspinatus tendon
(structures within the coracoacromial space)
What are the pre-disposing factors for Subacromial pain syndrome?
Poor posture
Scapular Dyskinesis
GH instability
Weak RC muscles
How does poor posture cause Subacromial Pain Syndrome?
Functionally reduces the size of the subacromial space
How does scapular dyskinesia cause Subacromial Pain Syndrome?
Altered scapular control reduces the length-tension relationship of the rotator cuff and causes altered scapulohumeral rythym
How does GH instability cause Subacromial Pain Syndrome?
Lax anterior capsule –> excessive anterior and superior translation of the humeral head –> cuff works harder to control –> overload reached quicker –> compression of subacromial space
How does rotator cuff weakness cause Subacromial pain syndrome?
Allows superior migration of the humeral head into the coracoacromial arch causing compressive damage
What are the clinical symptoms of Subacromial Pain Syndrome?
- Pain on super-lateral shoulder
- Pain when elevating and LR shoulder
- Pain on overhead movements
- Pain lying on the shoulder
- Shoudler feels weak
- Incidious onset with phases of settling then worsening
What intrinsic factors can cause subacromial pain syndrome?
Acute cuff overload or chronic overload resulting in tendinopathy (with small or large tendon tears)
What are the types of partial rotator cuff tears?
supraspinatus tendon most commonly effected
- inferior surface more likely to tear as higher tensile loads
What is a massive cuff tear?
affects both supraspinatus and infraspinatus tendons - extending for more than 5cm
