What are the stages of the Transtheoretical Model and Stages of Change?
Precontemplation, Contemplation, Preparation, Action, Maintenance and Termination
What is precontemplation?
“I won’t” or “I can’t
No intent to change the behavior in the next 6 months
Client has no personally motivated reason to change
Ignorance is bliss!#
“I know I should quit drinking so much caffeine
Validate client’s lack of readiness to change “The decision is yours. This is your life.” Encourage self-exploration - don’t try to push action
Be careful not to sound judgmental in your words or tone of voice
Three D’s of contemplation: Don’t Know How, Demoralization, Defending
Don’t Know How: Hearing that exercise is beneficial and it’s benefits but doesn’t know how being sedentary ca hurt your health.
Demoralization: feeling uncertain about changing
Defending: denying behavior is risky.
Types: (Turning Inward) Withdrawing: withdrawing from help, not accepting help
Going Silent: Not talking about the behavior that is affecting a person’s health
Internalizing: Where the patient or client blames problem on themselves
(Turning Outward)
Projection: Blaming others for their behavior
Displacing: Redirecting distress to to a substitute object or person that’s safe to attack.
What is contemplation?

Validate that client is not quite ready to make a change Clarify that it is the person’s decision

What is preparation?

Person intends to take a forward step in the next month
Offers a “window of opportunity
Ready to “test the waters
Example: “I bought some decaf tea to drink during my night shift instead of my coffee.
Preparation Goals
Problem solve a plan
What ideas does the client have?
What are potential obstacles? Brainstorm and add some of your ideas.
Always have the client choose which ideas to further explore. What are some realistic small steps?
Words of caution might be helpful. What type of support system does your client have?
