- governance, leadership, and workforce
- communication and language assistance
- engagement, continuous improvement, and accountability

What do you CALL the problem?
What do you think CAUSED the problem?

What is the Patient Care Process?
Collect: collect info about pt to understand relevant med history
Assess: assess info collected and analyze pt therapy to identify and prioritize problems
Plan: create a plan that is evidence-based and cost-effective
Implement: implement plan with pt
Follow-up: Monitor and Evaluate
What is essential pt info + sometimes important info
Essential: pt name, dob, allergies, other meds/conditions, who the prescriptions for
Sometimes: family history, social history (tobacco, alcohol, caffeine, diet, exercise)
three key concepts for counseling
- indication: what are they taking med for? what problems or symptoms is this med for?
- direction for use
- expectations: what were they told to expect from med?
