What are the types of osteomyelitis?
Haematogenous, non-haematogenous (direct inoculation and local invasion)
What is important about the looped capillaries in the metaphysis?
Bacteria can get stuck here
How do bacteria infections from the looped capillaries?
infection can’t get through growth plate so expands to sub periosteal space and shaft of bone
What is different about bacterial infections in children?
there are blood vessels that travel across growth plate, so infection can spread across here
What are the main causes of osteomyelitis?
Vast majority are S. aureus then Strep. pyogenes
Why is staph aureus a major pathogen?
staph has many virulent factors which allow it to infect joints
In Infants what are the most likely causes of osteomyelitis?
Group B strep and H. Influenzae
How is osteomyelitis diagnosed?
- Clinical signs
- X-ray
- Blood culture
What is the treatment for osteomyelitis?
Flucloxacillin 1st generation best for gram +ve
What is treatment for bacterial resistant osteomyelitis?
Vancomycin
What is associated with chronic osteomyelitis?
Suboptimal initial treatment
