W25-L3: Hip complex Flashcards
What is the line joining the greater and lesser trochanter on the anterior surface?
intertrochanteric line
What is the line joining the greater and lesser trochanter on the posterior surface?
intertrochanteric crest
Where does the head of femur article with the acetabulum?
The roof of the acetabulum (thickest cartilage)
What are the intra-arcticular strucutres?
- Labrum: deepens acetabulum
- Ligament of head of femur: only directs branch of obturator artery to femoral head
There are 2 systems of the trabecular system that meet at the hip inferior and superior. What are they?
Superior: Vertical and arcuate
Inferior: medial and lateral
Where does the line of gravity pass relative to the hip?
posterior
What is the close packed position?
where articulate surfaces meet the best and ligaments are torte
When is close packed position for the hip?
extension, abduction & medial rotation
What is open packed position for the hip?
Flexion, adduction, lateral rotation
What is the ligamentous support for the hip?
- Strong iliofemoral anterior
- Weaker behind (pubofemoral and ischiofemoral), don’t attach to femur directly
What are the retinacular fibres?
reflections of fibrous tissue from capsule to the femoral neck which contain retinacular vessels
What do the retinacular vessels supply?
Head of femur
What are the range of flexion and extension at the hip?
Flexion: 120
Extension: 10-20
What do the cuff of fixator muscles do?
Stabilise the head of femur by pulling it into the socket
What is the primary role of gluteus medius and minimus?
Maintain the position of pelvis in a coronal plane
Which nerve innervates the hip?
Branches of femoral and obturator
Where does referred pain from the hip go?
Obturator nerve
Where does referred pain to the hip come from?
Lumbar spine and sacro-iliac joint
What is the blood supply of the hip?
anastomoses between branches of medial and lateral femoral circumflex
Where does dislocation of the hip generally occur?
In the posterior direction
What is the neck-shaft angle in adults?
135 degrees
What happens in fractured NOF?
Blood supply especially retinacular vessels are susceptible
