Pathophysiology of BPPV.
A
Calcium Carbonate Crystals - Otoconia become displaced into the semicircular canals (especially posterior semicircular canal) and disrupt normal flow through canals.

Aetiology of BPPV.
A
- Viral Infection.
- Head Trauma.
- Ageing.
- Idiopathic.

Clinical Features of BPPV (5).
A
- Triggered by Movement e.g. in bed.
- Last around a minute before symptoms settle.
- Over Several Week Clusters.
- Asymptomatic between attacks.
- No tinnitus/hearing loss.

A
Dix-Hallpike Manoeuvre.

What is the Dix Hallpike Manoeuvre? (2)
A
- Move patient’s head in a way that moves endolymph through canals and triggers vertigo.
- Positive : Rotational Nystagmus (towards affected ear) and Vertigo.

A
- Epley Manoeuvre.
- Brandt-Daroff Exercises.
- ?Medication = Betahistine.

What is the aim of the Epley Manoeuvre?
A
Move crystals in the semicircular canal into a position that does not disrupt flow.
