hemiazygos vein pathway

A

Arises on the left side, by the junction of the left subcostal and ascending lumbar vein
It ascends on the left side of the vertebral column, posterior to the thoracic aorta.
At the T9 vert. it crosses to the right, posterior to the aorta, thoracic duct and oesophagus and joins the azygos vein.

Accessory hemiazygos vein

A

begins at the medial end of the 4th and 5th intercostal space and ascends on the left side of the vert. column (T5-T8).
It crosses over at T7/8, posterior to the thoracic aorta/ thoracic duct, where it joins the azygos vein.
Sometimes the hemi- and accessory hemiazygos vein join and open together in the azygos vein
