Issue: Vehicle Accident
Immediate Actions
- Check and provide medical treatment for any injuries
- Is apparatus available for use?
- Place out of service
- Tow truck through Duty Apparatus Officer
- Ensure personnel do not discuss with anyone except
- PD
- FD officials
- FX County Risk Management
- Representatives of the County Claim Services Company
- Consider moving driver to a non-driving position based on events
Investigate
- Obtain other driver’s information
- Full name
- Address
- Insurance information
- Policy number
- Name and contact information of all witnesses
- Photos
- Review driver’s personnel file
- Driver qualification forms
- Vehicle log book for deficiencies
- Terrain, weather, etc.
- Pass on book
- VMS M5
- Consider fitness for duty for driver
Notify
- Battalion Management Team (BC and EMS Supervisor)
- Safety Officer
- Uniformed Fire Officer
- Police
- Volunteer Chief/President if applicable
- Duty Apparatus Officer
- Staffing coordinator (Ops406) if employees are not able to work
- Place on administrative leave, update telestaff/FOCUS
- CISM if needed
Document
- Supervisors notes (names, date, time)
- Station log book
- Incident report
- VMS M5
- Vehicle log book
- Pass on book
- If injuries (SOP 02.03.01)
- Injury packets
- FRD-072-FX County vehicle accident report
- FRD-317-Narrative forms from
- Entire crew
- Any witness
- Police report
- If needed
- Risk 05-Citizen Property Damage report
- FRD-075-Significant Incident report
- Risk 03-Property Loss Notice (if portable equipment damaged)
Follow up
- Recommend progressive discipline to BC as appropriate
- Delegate training for the shift to LT or Master Technician on SOPs, station policies, and ways to prevent future occurrences
- Review training in 3 working days, have the training delivered in 6 working days
- Accident review board recommendation for training
- Driver improvement plan
