Employee Injury required steps:

A

Issue: Employee Injury

Immediate Actions

  • Provide medical treatment, call for EMS response if needed
  • Place unit out of service
    • Adjust staffing to maximize in service units
  • Provide employee with injury packet
  • Contact employee’s family as a courtesy

Investigate

  • Written statements from all parties and witnesses
  • Inspect work area and equipment
  • Photos
  • Actions leading up to event
  • Review SOP 02.03.01 Personal Injury Reporting

Notify

  • Battalion Management Team (BC and EMS Supervisor)
  • Uniformed Fire Officer
  • Safety Officer
  • Staffing coordinator (Ops406) if employees are not able to work
    • Update telestaff/FOCUS

Document

  • Supervisors notes (names, date, time)
  • Station log book
    • Full legal name
    • Location of injury
    • Incident number/report if applicable
    • Brief description of how injury occurred
  • Risk Management online claim reporting (www.claimonline.fairfaxcounty)
  • Employer’s First Report of Accident on-line
  • FRD-071 Employee’s Supervisor Report of Workplace Related Injury/Illness
  • FRD-074-Medical Status Form (if applicable)
  • FRD-091 Therapy Report Form (if applicable)
  • FRD-075-Significant Incident Report by end of 24 hour shift (if applicable)

Follow up

  • Recommend progressive discipline to BC as appropriate
  • Repair/replace faulty equipment
  • Delegate training for the shift to LT or Master Technician on SOPs, station policies, and ways to prevent future occurrences
    • Review training in 3 working days, have the training delivered in 6 working days
  • Maintain contact with injured employee at least weekly until return to full duty
