Citizen Injury required steps:

A

Issue: Citizen Injury

Immediate Actions

  • Provide medical treatment
  • Call for EMS response

Investigate

  • Citizen name, address, phone number, statement
  • Witness statements with name, contact information
  • Photos
  • Review SOP 01.09.11 Liability Incident Reporting

Notify

  • Battalion Management Team (BC and EMS Supervisor)
  • Safety Officer
  • Uniformed Fire Officer

Document

  • Supervisors notes (names, date, time)
  • Risk 06-Citizen Injury Report (Risk 07 in the SOP, Risk 06 on the form)
  • FRD-317-Narrative Statement form from all witnesses and employees
  • FRD-075-Significant Incident Report by end of 24 hour shift (if applicable)
  • FRD-072-If caused by equipment or hose failure
  • Log book, Citizen full legal name and where injury occurred

Follow up

  • Recommend progressive discipline to BC as appropriate
  • If caused faulty equipment
    • Turn over to Safety Officer
    • Obtain loaner
  • Delegate training for the shift to LT or Master Technician on SOPs, station policies, and ways to prevent future occurrences
    • Review training in 3 working days, have the training delivered in 6 working days
