Define Varicocoele
The abnormal dilation of the veins in the internal spermatic veins and pampiniform plexus
Where does the pampiniform plexus drain blood from?
The testes
Which testes is more commonly associated with varicocoele? And why?
The left testes.
- Because of the angle that it drains into the left renal vein.
- Lack of effective valves between the left renal vein and the left testicular vein.
- Increased reflex from compression of the renal vein by the SMA and aorta.
What are the risk factors for varicocoele?
- Tall men
- Low BMI
- Fhx
All weak risk factors
What is the epidemiology of varicocoele?
- Less common in pre-puberty
- Can be associated with infertility
- 90% in left side
What are the presenting symptoms of varicocoele?
Often asymptomatic and an incidental finding
- Heavy scrotum
- Scrotum feels like bag of worms
What are the signs on physical examination of varicocoele?
- Side affected will hang lower
- Valsalva manouevre will increase dilation
- Cough impulse
What must you ask patients to do when examining them for varicocoele?
Patient must be standing.
Lying down will reduce swelling
What are the appropriate investigations for varicocoele?
- Semen analysis for infertility
