Define epididymitis
It is the inflammation of the epididymis
Define orchitis
It is the inflammation of the testes
What are the risk factors for epididymitis and orchitis?
- Unprotected sexual intercourse
- Bladder outflow obstruction
- Instrumentation of urinary tract
What are the causes of epididymitis and orchitis?
Mostly bacterial
Younger patients = Chlamydia trachomatis
Older patients = Enterobacter (enteric pathogens)
What is the epidemiology of epididymitis and orchitis?
Most commonly between ages of 20-39yrs
What are the presenting symptoms of epididymitis and orchitis?
- Painful/tender/swollen testes/epididymis
- Dysuria
- Sweats or fever
What are the signs of epididymitis and orchitis on physical examination?
- Swollen/tender testes/epididymis
- Scrotum may be erythematous
- Pyrexia
- Walking may be painful
- Cremasteric reflex may be painful
What are the appropriate investigations for epididymitis/orchitis?
- Urine dipstick and MCS
- Bloods - WBC (high), CRP (high)
- Ultrasound (duplex)
What is the management for epididymitis/orchitis?
- Antibiotics
- Analgesia
- Abscess drainage if needed
What are the complications for epididymitis/orchitis?
- Abscess
- Fertility issues
- Chronic pain
- Testicular atrophy/obstruction
What is the prognosis for epididymitis/orchitis?
- Rapid recovery with treatment
