Define renal cell carcinoma
A malignant growth originating in the kidney
Which part of the kidney do RCC’s originate from usually?
The epithelial cells of the proximal convulated tubule (the cortex)
What are risk factors for RCC?
- Smoking
- Male
- Fhx of RCC
- Polycystic kidney disease
- von Hippel-lindau disease
Mutations in which chromosome is linked to RCC?
mutations in Chromosome 3
What does a mutation in the chromosome linked to RCC result in?
Increased IGF-1 pathway activity resulting in:
- Dysregulated cell growth
- Angiogenesis
What is the epidemiology of RCC?
Uncommon
3% of all adult cancers
What are the presenting symptoms of RCC?
- Usually asymptomatic until very late
- Blood in urine
- Pain in flank
What are the signs of RCC on physical examination?
- Palpable abdo mass
- Flank pain
- Hypertension
- Left sided varicocoele
- fever
- weight loss
What are examples of paraneoplastic syndromes associated with RCC? What do they result in?
Paraneoplastic syndromes are caused by tumour generating hormones. Examples are: -EPO - polycythaemia -Renin - hypertension -PTHrP - hypercalcaemia -ACTH - cushing's syndrome
What are the appropriate investigations for RCC?
- Bloods - increased RBC, calcium, ALP (bony mets)
- Urinanalysis - haematuria
- USS of kidney
- CT of kidney
