Define glomerulonephritis
The immune mediated inflammation of the glomeruli and nephrons of the kidney.
What are the risk factors for glomerulonephritis?
- Group A haemolytic streptococcus
- HIV
- SLE
- Drugs - gold
- Colorectal cancer
What causes glomerulonephritis?
Glomerular injury may be caused by inflammation due to leukocyte infiltration, antibody deposition, and complement activation
What is nephrotic and nephritic syndrome?
- Nephrotic syndrome is the increased permeability of the glomerulus = LOSS OF A LOT OF PROTEINS IN URINE
- Nephritic syndrome is the thin glomerular basement membrane with pores = LOSS OF A LOT OF BLOOD
What is the epidemiology of glomerulonephritis?
25% of patients with chronic renal failure have this
What are the presenting symptoms for glomerulonephritis?
- Haematuria
- Polyuria or oliguria
- Symptoms of acute or chronic renal failure
- History of recent infection
What are the signs of glomerulonephritis on physical examination?
- Hypertension
- Haematuria
- Oedema
- Low urine output
What are the investigations for glomerulonephritis?
- Urinanalysis
- Bloods - FBC, U&Es and lipid profile
- Antibodies (ANA and anti-GBM)
- USS of kidneys
- Renal biopsy
What is the triad for nephritic syndrome?
- Haematuria
- Hypertension
- Proteinuria
Why is there hyperlipidaemia with hypoalbuminaema?
The liver tries to compensate for the lack of protein in the blood by increasing the production of lipids.
