What are common indications for the use of vancomycin?
- Endocarditis
- Antibiotic resistant colitis

What organisms does vancomycin target?
Gram-positive organisms
What is the mechanism of action of vancomycin?
Inhibits growth and cross-linking of peptidoglycan chains, inhbiting synthesis of cell wall of gram positive bacteria
What are the most important side effects of vancomycin?
- Thrombophlebitis
- Anaphalactoid reactions
- Allergy
- Nephrotoxicity
- Ototoxicity
- Blood disorders - neutropenia, thrombocytopenia
What drugs can vancomycin interact with?
- Aminoglycosides
- Loop diuretics
- Ciclosporin
How is vancomycin normally administered?
IV - very poor at crossing lipid membranes
When would vancomycin be given orally?
C. diff infections
Why must vancomycin be given slowly IV?
To avoid analphylactoid reaction
What is the therapeutic range for vancomycin?
10-15 mg/L
How would you monitor vancomycin?
- Measure trough levels, and adjust vancomycin dose to maintain in therapeutic range
- Signs and symptoms
- Blood markers - CRP, WCC
- Daily U+E’s and FBC
