40 yo. Based on clinical presentation need abdominal imaging, but what to do first?
All women of childbearing age first require pregnancy testing (eg, serum β-hCG level).
This is not only to guide the choice of imaging modality, but also to evaluate the risk for ectopic pregnancy.
UW. in case: 40yo. Temperature is 37.9 C (100.2 F), blood pressure is 110/80 mm Hg, pulse is 110/min, and respirations are 21/min. The abdomen, especially in the lower quadrants, is tender on palpation with mild guarding.
What is good and what is alarming?
Good: HD stable
Alarming: fever, tachycardia, guarding
this case warrants imaging, but age 40 –> do hCG

why ectopic can cause fever?
ectopic rupture and intraabdominal bleeding (which itself can cause low-grade fever due to peritoneal inflammation).

transvaginal ultrasonography is performed next because it is best for determining pregnancy location and evaluating the adnexa, all while avoiding exposure to teratogenic ionizing radiation.

negative pregnancy test makes ectopic pregnancy unlikely.
Other diagnoses (eg, appendicitis, diverticulitis, tuboovarian abscess) require more consideration, and the choice of imaging modality will depend more on the suspected diagnosis (eg, CT scan of the abdomen and pelvis for suspected diverticulitis, pelvic ultrasonography for suspected tuboovarian abscess)

hemodynamically unstable due to suspected intraabdominal bleeding (eg, ruptured ectopic pregnancy).

When Dx has been established, to monitor disease progression.
can be performed after the diagnosis and initiation of treatment (eg, antibiotics for pelvic inflammatory disease); this approach is helpful for monitoring disease progression and the need for additional intervention. It is inappropriate when the evaluation is incomplete, as in this patient.

determine pregnancy status because pregnancy status affects the type of antibiotic prescribed if a urinary tract infection is identified.!!!!!!!!!

Women of childbearing age with acute abdominal pain require immediate pregnancy testing because determination of pregnancy status is needed to guide additional evaluation (eg, choice of imaging modality) and management.
