- Bacterial colonisation
- E.Coli contain fimbrae - Inc adherence to epithelium, which inc. their survival
Ix
pyuria +/- bacteriuria
- Urinalysis:
- Leucocyte esterase (released by WBC)
- Nitrites (pathogens convert nitrates to nitrites)
Mgx of uncomplicated UTI
-Cystitis:
Abx:
Trimethroprim and nitrofurantoin
- Pyelonephritis:
fluoroquinolone (e.g. ciprofloxacin)
Mgx of complicated UTI
- Cystitis:
fluoroquinolone - pyelonephritis/urosepsis:
Co-amoxiclav
