- Ischemia
- Malignant Htn
- Wegener Granulomatosis
Urinary Casts
Significance?
Presence of urinary casts indicates that hematuria / pyuria is of RENAL (vs bladder) origin.
WBC Casts
Seen In (3)?
SEEN IN:
- Acute Pyelonephritis
- Tubulointerstitial Inflammation
- Transplant Rejection
Eosinophilic Casts (resembling thyroid tissue)
Seen In?
SEEN IN:
- Chronic Pyelonephritis (“Thyroidization” of kidney)
Fatty Casts (“oval fat bodies”)
Def’n, Seen In?
Dead PCT cells loaded with lipids (from lipid catabolism in PCT).
SEEN IN:
- Nephrotic Sx
Granular Casts (“muddy brown”)
SEEN IN:
- Acute Tubular Necrosis / INTRINSIC Acute Renal Failure
Hyaline Casts
Nonspecific. Can be a normal finding, often seen in concentrated urine samples.
Renal Epithelial Casts
Significance, Seen In?
Signify tubular damage.
SEEN IN:
- Acute viral or toxic renal injury
Waxy Casts (accompanied by very low urine flow)
Seen In (2)?
SEEN IN:
- Advanced renal disease
- Chronic Renal Failure
