Internuclear Opthalmoplegia (INO)

Location Affected, Def’n, Mech, Assoc, Seen In (2)?

A

Damage to MLF.

Disorder of LATERAL conjugate gaze: affected eye can’t adduct, nystagmus in unaffected eye.

MECH: LACK OF COMMUNICATION-
when Abducens nucleus activates ipsilateral Lat Rectus,
contralateral Oculomotor nucleus fails to stim Med Rectus.

ASSOC:
- Demyelination

  • MS (bilateral)
  • Stroke (unilateral)

BILATERAL small pupils that accommodate (reduce in size when patient focuses on near object) but don’t react to light (constrict).

SEEN IN:
- Tertiary Syphilis (Neurosyphilis) + its resulting pathologies (Tabes Dorsalis)
