Tx of MSK impairments in persons w/ neurologic pathology Flashcards
t/f, a static splint or orthotic can help gain ROM
false, won’t help gain range
What are conservative treatment options for contractures?
stretching and exercise, dynamic splinting, serial casting
What are common precautions common to all tenotomies?
initially nwb, initiation of WB varies
start with AROM
ask when PROM, stretching, ROMin wb and strengthening will be able to be started
movement retraining in new ROM
*BACK OFF IF YOU CAUSE PAIN
T/f, you want to allow the pt to have a posterior pelvic tilt in a wc after a hamstring release
false, do not allow posterior pelvic tilt
t/f, weakness is a consequence of a plantar flexor release
true! want to focus on strengthening when appropriate after a PF release
What are the different types of osteotomy?
rotation and wedge
How is a wedge osteotomy different from a rotation osteotomy?
A wedge shortens bone a little bit while a rotation osteotomy cuts and turns the bone to fix it
What are the different types of limb shortening?
bone resection, epiphysiodesis, epiphyseal plate stapling
What is the purpose of a epiphysiodesis surgery?
slows rate of growth of long leg-> to allow short leg to catch up
What are the limb lengthening surgeries?
ilizarov procedure, debastiani
t/f, healing time of limb lengthening surgery depends on extent of lengthening
true, each 1 cm (o.4in) requires 36 days of healing
What are the advantages and disadvantages of epiphysiodesis?
advantage- shorter healing time
disadvantage- timing is CRITICAL, causes short stature, fewer risks, less precise
What are the advantages and disadvantages of limb shortening?
advantage- more precise
disadvantage- more invasive, longer recovery time, may cause short stature
What are the advantages and disadvantages of limb lengthening surgery?
advantage- very precise, does not reduce stature
disadvantage- MOST invasive, long recovery (follow-up, rehab)
What are the general guidelines for limb length? (measurements and tx options)
less than 2cm- orthotic lift
2-6cm: epiphysiodesis or shortening procedure
greater than 6cm: lengthening procedure, staged or combined with epiphysiodesis
