Transplantation Flashcards
what is the need for kidney transplant?
survival benefit, QOL increases and major cost benefit (after first year especially)
what is the perfect match for transplantation
genetically identical mice
what is the universal donor
O
what are the 3 important HLA types
HLA A and HLA B and HLA DR (type 2)
why do more women get more antibodies than males?
pregnancy
where do you get antibodies from
previous transplant
what happens if there is no rejection
lots of infections- cytomegalovirus (herpes), recurrent UTI, pneumonitis jirovecii- co trimoxazole, BK virus- transplant disfunction, EBV post transplant,
what type of malignancy is associated with transplantation
squamous cell carcinoma§
what types of rejections are there
hyperacute- minutes due to mismatch- remove kidney
acute- usually early
chronic- immunological and vascular deterioration
what do we do to reduce immunosuppression after transplant
steroids, CyA, MMF Tacromilus antibodies
what do tacrolimus and cyclosporin do
reduce NK cells and cytotoxic T cell activation
what are side effects of cyclosporin and tarcolimus
renal dysfunction, hypertension, diabetes, tremors
what do azathioprine and mycophenolate do
block purine synthesis
what does azathioprine interact with
allopurinol- V V BAD- leukopenia and anaemia
what are side effects of steroids
osteoporosis, weight gain, infection and diabetes
