- alteplase

- streptokinase

mechanism of action:
A
- alteplase + reteplase + tenecteplase → recombinant t-PA, catalyze conversion of plasminogen → to plasmin, degrades clots
- streptokinase → from Streptococci → rarely used
- urokinase → from human neonatal kidney cells, cleave arginine-valine bond of plasminogen → to yield active plasmin

indications:
A
- lysis of clots in MI, thrombotic stroke, pulmonary embolism
- 2-6h therapeutic window
- urokinase → ONLY approved for pulmonary embolism

CONTRAindications:
A
- PREGNANCY
- healing wounds
- history of cerebrovascular accident
- brain tumor
- head trauma
- intracranial bleedings
- metastatical cancer

side effects:
A
- hemorrhages (less with t-PA)
- arrhythmias
- hypersensitivity → streptokinase
