- widely distributed
- excreted in urine

- peptic ulcer, stress ulcer (PPIs are more effective)
- reduce nocturnal acid secretion, accelerate healing, & prevent recurrences
- GERD (PPIs are more effective)

- generally well tolerated
- cimetidine → endocrine effects = gynecomastia, galactorrhea
- CNS symptoms (if given IV)
- drug interactions

- pantoprazole
- rabeprazole

pharmacokinetics of PPIs:
A
- ORALLY
- some are available as IV
- converted to active drug in parietal cells → may need 3-4 days of treatment to achieve full effectiveness
- enteric coated to prevent acid inactivation
- metabolized in liver → t1/2 = 1-2h

- GERD
- peptic ulcer
- non-ulcer dyspepsia
- prevention of stress-related mucosal bleeding
- Zollinger-Ellison syndrome
- erosive esophagitis

side effects of PPIs:

- diarrhea, abdominal pain, nausea
- headache
- hypergastrinemia
- increased risk of carcinoid & colon cancer
- low vit. B12
- drug interactions → with drugs that need acidic environment
