Wells rule for DVT
A
1 point for each
- Active cancer
- Paralysis, bed ridden, immobility
- Bedridden for > 3 days, major surgery in 12 weeks
- Tenderness along DVT areas
- Calf swelling (>3mm asymmetry)
- Asymmetric pitting edema
- Collateral superficial veins
- Prior DVT

A
3 or more = high risk
1-2 = intermediate risk
0 or negative = low risk

12 Exclusion criteria for thrombolytics
A
- Intracranial hemorrhage on CT or history ever
- Clinical presentation of subarachnoid hemorrhage
- Neurosurgery, head trauma, or stroke in past 3 months
- Uncontrolled HTN (185/110)
- Known intracranial AVMs, neoplasm, or aneurysm
- Active internal bleeding
- Suspected/confirmed endocarditis
- Platelets < 100k
- Heparin in the last 48 hours with PTT elevated
- Current use of oral anticoagulatnts
- INR > 1.7
- Glucose < 50
