Q

Five Key Components (ADPIE)

A

1) Assessment
- Collect data
- Medical history
- Measure vital signs
- Head to toe assessment
- Ask questions about their lifestyle
2) Diagnosis
- Response of what is happening to the patient.
- The diagnosis reflects not only the patient in pain,
but that the pain has caused other problems such as
anxiety, poor nutrition, or has the potential to cause
complications.
3) Planning/Outcome
- Outcome identification is where you establish to go
or your expected outcome for your patient.
- You will set short and long range goals for the
patient.
4) Implementation
- Where you take action on planned interventions.
- The care they provide will be based on the
assessment data and the planning care they
developed in the previous step.
5) Evaluation
- Reassess the patient (remember that anything can
change at any moment)
