When taking PO bisphosphantes, what should we teach client?
- Take in AM
- Take on EMPTY STOMACH
- FULL glass of water
- Stay UPRIGHT for 30-60 mins
Why does client need to keep standing for 30-60 mins after taking a -dronate?
it is extremely irritating to the esophagus and we don’t want any reflux
A/E of -dronate
- Heartburn
- Abdominal Pain
- Diarrhea
- Hypocalcemia
- Osteonoecrosis of the Jaw (ONJ)
Benefits of raloxifene (besides increasing bone density).
Decreases cancer risk
BBW for raloxifene
Thromboembolism
Stroke
How can client minimize risk of Thromboembolism or Stroke while taking raloxifine?
Avoid prolonged inactivity, as can happen when traveling
Patients should discontinue raloxifene at least 72 hours before prolonged immobilization (ie: surgery)

What is our goal when treating osteoporosis?
We want to block the osteoclasts (that chew up bones)
What are the Osteoporosis meds we discussed?
- Bisphosphonates (-dronate)
- Raloxifene
Which routes are -dronate given?

Why do -dronate cause Hypocalcemia?
They pack the calcium back into the bones, leading to low calcium in the blood
Why drink a full glass of water w/ -dronate
So it won’t get stuck in esophagus
Remember it’s EXTREMELY IRRITATING to the esophagus
MOA: raloxifene
- mimics estrogen in the bones
- blocks estrogen in breasts & uterus
A/E of raloxifene
- Hot Flashes
- Leg Cramping
- Edema (weight gain)
