List and describe the 3 steps of drug therapy assessment
- Collection of patient-specific information
- Assessment of the patient’s drug-related needs
- Identification of drug therapy problems
Collection of patient-specific information
The practitioner collects relevant patient-specific information to use in decision making concerning all drug therapies
Assessment of the patient’s drug-related needs
The practitioner analyzes the data to determine if all the patient’s medications are appropriately indicated, the most effective, the safest possible, and the patient is able and willing to take the medication as intended.
Identification of drug therapy problems
The practitioner analyzes the data to see if any drug therapy problems are present.
Questions to ask to look for DTP regarding indications
- Are there any missing medications?
* Are there medications that are unnecessary? Missing an indication?
Questions to ask to look for DTP regarding effectiveness
- Is each drug individually appropriate/effective for the problem?
- Are these drugs in combination appropriate for the problem?
- Are all surrogate markers at goal?
- Is the dosage form appropriate to treat the condition?
- Is the dose high enough?
- Is the dosing frequency too far apart?
- Is the medication being administered at the optimal time?
- Is the duration of therapy too short?
Questions to ask to look for DTP regarding safety
- Are the doses appropriate based on renal or hepatic function and patient age?
- Is the dosing frequency too close together?
- Is the duration of therapy too long?
- Are there any serious drug interactions among all the drugs?
Questions to ask to look for DTP regarding adherence
• Are the medications being filled at appropriate intervals based on refill history
