Hematemesis
bloody vomitus (lower in GI system), Appears fresh, bright red blood or “coffee grounds”
melena
black, tarry stools; caused by digestion of blood in GI tract (went through digestive system), Black appearance—due to iron
occult blood
undetectable by appearance; small amounts of blood in gastric secretions, vomitus, or stools
occult blood is detectable by
guaiac test
clay-colored
issue with gallbladder or liver
common causes of UGI bleeding
Esophageal varices, BLUNT AND PENETRATING TRAUMA, liver disease, peptic ulcer disease, Drugs
o Aspirin, NSAIDs, corticosteroids, Plavix, Coumadin
Þ Sickle-cell anemia, DIC
Þ Stress-related mucosal disease (SRMD)
o Also called physiologic stress ulcers
diagnostics for UGI bleeding
ENDOSCOPY IS PRIMARY TOOL
-angiography
-labs (CBC, BUN, electrolytes, PT, PTT, liver
-GUAIAC
Massive upper GI hemorrhage is defined as
1500 mL of blood
if a patient with a hemorrhage stops bleeding it means
CLOT HAS FORMED AT SITE AND THEY CAN TAMPONADE OFF, BUT IT CAN BLEED AGAIN
Emergency Assessment and Management
o Immediate physical examination with focus on (can go into hypovolemic shock)
o Indwelling urinary catheter for assessment of hourly output (NEED TO WATCH THE KIDNEYS)
o Hemodynamic monitoring for accurate blood pressure assessment (A-line)
o Central venous pressure line for assessment of fluid volume status
o Supplemental oxygen
o Abdominal examination
o FLuids
o Endoscopic hemostasis therapy
abdominal exam with UGI bleeding
- Presence or absence of bowel sounds
- Tense, rigid, boardlike abdomen: may indicate perforation and peritonitis
fluids for UGI bleeding
volume is based on what they need
-Isotonic crystalloid- Lactated Ringers
-Volume replacement (based on what they need)
· Whole blood, packed red blood cells, fresh frozen plasma
Endoscopic hemostasis therapy
-First-line therapy of upper GI bleed, locate bleeder and BURN IT
-Goal: to coagulate or thrombose the bleeding vessel
Endoscopic hemostasis therapy interventions
NPO, HOB up, hydrate
drug therapy for UGI bleed
-PPI, H2
-injection with epi during endoscopy for acute hemostasis (vasoconstricts quick)
