How do you determine the suction catheter size (for closed suction) in adults?

What is Closed Suction therapy procedure?
A
- Auscultate the Pt and assess for indications to suction
- Increase O2 by 10-15% (100% for adults)
- Check broslow tape for the appropriate size of the suction catheter
- Insert the suction catheter until resistance is met and pull back a little
- Apply suction and pull back
- Assess the COCA of secretions and if another suction pass is required
- Check the Pt vitals before going in for another (and instill saline if you do)
- Auscultate after suctioning and return O2 levels back to what they were prior

What is the timeline for suction procedures to be performed as clean vs sterile?
A
- 0-72hrs = Sterile
- 72hrs+ = clean

On neo and Ped Pts with a Trach, How do you determine open suction depth?
A
Its based on the size of the Trach tube/inner cannula.
- Ask the prof for another trach to so you can measure it
- Catheter size should be based on brosolow tape

When would you perform inline suction on a neo or ped population?
A
Done on a trach patient or ETT pt where an inline catheter isn’t present
- Check for patient vitals and auscultate (most likely for crackles for suctioning)

What is open suction procedure for neos/peds?
A
- Turn the suction on to -100mmHg
- Insert the catheter to appropriate depth
- Apply suction and pull out
- Inspect the catheter for COCA of the secretions
- Always clear the catheter w/saline solution before going for another pass

What is open suction procedure for adults?
A
- Gather either size 12 or 14 Fr suction catheter
- Turn the suction pressure to -100mmHg
- Preoxygenate patient at 100% for 2 mins
- Have another person help to disconnect the vent from Pt ETT
- Advance the suctinon catheter until resistance is met and move back a few cm
- Apply suction pressure and pull the catheter out slowly
- Comment on the color, odor, consistency amount
- If large amount, advance for a second pass
