What is Stridor?
Large airways obstruction (larynx, trachea, main bronchi) producing wheeze
Causes of Stridor in children? (4)
- Infection- epiglottitis
- Foreign body
- Anaphylaxis
- Other- burns
Causes of Stridor in adults (4)
- Neoplasms- larynx, trachea, main bronchi
- Anaphylaxis
- Trauma
- Others-
What investigations would you do for Stridor? (5)
- Laryngoscopy (beware in acute epiglottitis)
- Bronchoscopy
- Chest Xray
- CT
5 Flow vol loop
How would you treat Laryngeal obstruction?
- Remove object
- High flow O2
- Cricothryoidotomy (incision made through Cricothyroid membrane to maintain ventilation)
- Tracheostomy (incision in wind pipe to facilitate breathing)
What is the Heimlich Manoeuvre?
Rapid thrusts in epigastric forces upward diaphragm movement and forced expiration
How would you treat a malignant arirway obstruction in stridor?
- Tumour removal- surgery, laser, photo therapy
- Tumour suppression- Stent
- Radiotherapy
- Chemotherapy/ Drugs
What is acute Anaphylaxis?
A type I hypersensitivity reaction. Histamine release from mast cells
What are the systemic effects of acute Anaphylaxis? (3)
- Life threatening airway obstruction/ breathing problem- stridor, wheeze, respiratory failure
- Skin/ mucosal changes- angioneurotic oedema, urticaria, flushing.
- Hypotension- shock
Common causes of acute anaphylaxis? (4)
- Food- nuts
- Venom- bee, wasps
- Drugs- penicillin, aspirin
- Others- latex
Treatment for anaphylaxis?
- Secure airway, Give High flow O2
- IM adrenaline (0.5mg)
- IV antihistamine
- IV corticosteroid
- Nebulised bronchodilator
- Endotracheal intubation if necessary
Prevention for anaphylaxis?
- Allergy avoidance
- Desensitisation
- Personal use of adrenaline
What causes snoring?
Relaxation of pharyngeal muscle, partially obstructing upper airway causing turbulent airflow i.e sound
What happens in sleep apnoea?
Relaxed pharyngeal muscle doesnt pull inwards, tongue rests backwards onto the roof of the mouth
Effect- full airway obstruction
Person gasps for breath, then goes back to sleep unaware of what happened. This repeats throughout the night.
Risk factors for sleep apnoea?
- Obesity
- Inflamed tonsils
- Acromegaly (too much growth hormone)
- Drugs
Others- retrognathia, neurological, oropharyngeal deformities.
