Pyridoxial-dependent Strep require Vitamin B6 (pyridoxial hydrochloride) for growth
NVS satellite around E.coli, Staph, Klebsiella, yeast, Enterobacter
(Should be suspected in blood cultures are positive, but sub cultures are negative
Pharyngitis
sore throat, fever, exudate
NO cough/runny nose
Scarlet Fever

Pharyngitis with rash
Strawberry Tongue (red with white pillae)
Impetigo

lesions/pustules
treated with Pen
Erysipelas
follows strep throat
raised demarkated margin, confined to face

bug invades genital tract -> endometriosis, bacteremia, TSS, necrotizing fascitis
Acute Rheumatic Fever (ARF)
Kids 6-15 yo, 2-5 weeks post Strep Throat
Inflammatory lesions involving heart, joints, tissue, CNS
“Polyarthritis”: migratory, multiple joints
Endocarditis, myocarditis, pericarditis
Lab Findings:
increased CRP, Increased ASO, Increased ESR
Increased Anti-DNAse, Anti-hyaluronidase
Associated with large amounts of M protein and capsule
Acute Glomerulonephritis (AGN)
Inflammation of renal glomerulus
10days-6 weeks post infection
Edema, hyertension, dark urine, hematuria, proteinuria
Lab Findings:
Increased ESR, Anemia
Increased Anti-DNAse, Anti-hyaluronidase
NO increase in ASO
Strep agalactiae
Screening at 35-37 weeks
CAMP test + (produces CAMP factor, Staph must produce beta-lysin)
Bacitracin Resistant
Enterococcus faecalis
UTI, wounds, abscesses
Resistant to Beta-lactams and aminoglycosides
Enterococcus is Bile Esculin +
Strep mutans
tooth decay
Strep pneumoniae
virulence = capsule
Pneumonia, otitis media, bacteremia
TX: Pen, Erythro, chloramphenoicol
Strep Pathogenicity- Hemolysins
SLS: Strep A, C, G (causes beta-hemolysis); lyses RBCs after binding to membrane
SLO: Supresses chemotaxis, neutrophils, inhibits phaygocytosis, oxygen labile
ASO TEST
Measures Anti-SLO to see if there was a recent Strep. pyogenes infection
