gram negative bacilli
Classification/Diagnosis
Grows on living cells/tissues only test by nucleic acid amplification tests Serotypes A-C (trachoma in the eye) D-K (urogenital) L1-L3 (lymphogranuloma venereum)
Pathology
obligate intracellular pathogen
infectious elementary body enters cells, replicates as reticulate body, destroys host cells, stimulates inflammatory response
No lasting immunity
Epidemiology
Direct mucosal contact with infected membranes of fluids
Congenital
most commonly reported infectious disease in US, 4 million new cases/year
Adolescents at high risk
Diseases/Symptoms
Same as gonnorrhea minus pharyngitis/DGI and plus neonantal syndrome
Chronic infection is asymptomatic and causes tubal infertility
lymphogranuloma venereum (inguinal lymphadneopathy, proctitis, fever, tenesmus, bleeding, rectal pain)
Treatment/Vaccination
Single-dose azithromycin or 7-day regimen of doxycycline
screening is essential
