Morphology
Gram negative rods
facultative anaerobe
Classification/Diagnosis
Oxidase negative motile non-lactose fermenter <2500 serotypes test by stool culture
Pathology
Crosses M cells
Causes apoptosis of macrophages
modifies vacuoles to support survival
Invades epithelial cells basolaterally/disseminates systemically
Enters epithelial cells directly or through dendritic cells
large infectious dose required
Epidemiology
Either 1st or 2nd most common enteropathogen in US
Typhoidal and non-typhoidal strains transmitted through fecal-oral route
common cause is contaminated poultry, eggs, raw milk, dirty water
Diseases/Symptoms
gastroenteritis within 1-3 days
enteric fever, bacteremia, endovascular infection, osteomyelitis, abscess
typhoid fever (fever, bacteremia, belly pain, rose spots, hepatosplenomegaly, GI bleeding, peritonitis)
Treatment/Vaccination
Rehydration
