Morphology
Gram negative diplocci
Classification/Diagnosis
In men - test by gram stain of urethral discharge
Thayer-martin media
In both PCR, nucleic acid amplification tests
Pathology
Attaches to mucosal cells via pili and Opa protein
Infects subepithelial space
lipooligosacchride stimulates inflammatory response
Por protein evades destruction by phagolysosome
Epidemiology
Direct mucosal contact with infected membranous fluids
6x-7x higher prevalence in adolescents
Diseases/Symptoms
In men - urethritis (dysuria, discharge, burning) and epididymitis (pain, swelling erythema of scrotum)
In women - mucopurulent cervitis (discharge, dysaperunia, bleeding, dysuria, pain)
Pelvic inflammatory disease (tubo-ovarian abscess, perinonitis, ectopic pregnancy)
dysuria-pyuria, fizhugh-curtis syndrome
In both - conunctivitis, anorectal infection, pharyngitis, sequelae of disseminated gonococcal infection (DGI, joint lesions, arthritis), Reiter’s syndrome (arthritis, conjunctivitis, urethritis, skin lesions)
Treatment/Vaccinations
Single-dose ceftriaxone, other cephalosporins
Resistance is increasing - spectinomycin for those allergic, treat for chlamydia if not ruled out
Screening is essential
