Situational Questions Flashcards
When removing batteries from a system what dangers are present and what can you do to control them?
Electrical hazard from terminals – Cover terminals with caps immediately to prevent injury
by shorting terminals.
Internal Battery acid may be present – follow correct PPE requirements and handle/dispose
of correctly.
Name 5 provisions if you are to be working on a substation or near HV apparatus?
Barriers
Rubber floor matts
Visors
Additional Permits
Extra personal in the vicinity
Specific HV tooling/testing equipment
Arc flash/flame resistant clothing
You are fault finding on an LV system and discover there may be a fault on the HV circuit. What steps would you take?
Ensure team safety and stop work immediately.
Move working party to safe location.
Phone the Authorising Engineer and discuss findings.
Clear and cancel the current AWP if it is unsuitable for the current situation.
Request relevant documentation/tools/PPE and accompany task along with an SAP/AT2+ technician.
How do you test for dead when working on an LV or Hydraulic system?
Electrical
Disconnect and isolate components/devices from electrical supply (Isolations as per AWP).
Using an approved and calibrated Voltage absence tester and proving unit…
Prove Voltage absence tester using proving unit.
Test for zero voltage at point of isolation and terminals stated in the AWP.
Prove voltage absence tester once again to confirm serviceability.
Also, prove at the point of work.
Hydraulic
Disconnect electrical supply to HPU and secure (isolations as per AWP).
Open appropriate valves/outlets stated on AWP to drain pressure from the system being worked on.
Allow the system to vent and atmospheric pressure to neutralise within the system and vapors to dissipate.
Use Manometer to confirm zero pressure of system.
What precautions must be taken when working on LV apparatus?
- The AT must be authorised on that platform and familiar with with the work being carried out.
- Where practicable, a screen and rubber matting must be used when adjacent to live parts.
- Use only approved and insulated tools.
- Wear the correct PPE for the task:- Consider Arch flash and flame resistant PPE, Glove protection, goggles/glasses, visors etc.
- Remove all jewelry and metal items.
- Consider whether those carrying out the work require supervision when carrying out the task.
Explain and/or demonstrate the Surrender procedure and how the Surrender record is to be completed.
- AT surrendering AWP will fill section 1. (his/her name, date & time) of the surrender and any comments regarding the work/testing at point of surrender.
- The AT receiving the AWP will either receive it face to face with previous AT or from safe custody.
- New AT will accept AWP/documents/keys etc. and ensure working party are fully briefed with the status and progress of work.
- New AT signs and dates section 2. and enacts the transfer of control with OC as usual.
If there are any limitations placed on the WTG who must the AT inform?
Operational Controller and Authorising Engineer.
When can an AT1 apply HV isolations?
Only under the supervision of an AT2.
What precautions and method would you take when approaching a WTG with ice on its blades?
- Remotely stop the WTG
- Yaw Nacelle of the turbine away from/to the opposite side of the entrance door on the transition piece.
What would you do if an AWP was lost during work?
- Stop all work, remove the working party and inform them it is unsafe to carry on working.
- Search for the AWP
- If the AWP is not found, contact the OC and AE and initiate the GP3 procedure (Special instructions).
- Once GP3 is enacted, a new AWP will be issued.
- Fill in the AWP as normal, walk through each isolation point, check isolations are in place and sign signature checkbox.
- Work can then be restarted.
When working under an AWP with isolations in place, you find the point of work still energised/live, what do you do?
- Stop work immediately, inform the working party to stop work and remove them to a safe area.
- Verify the correct isolations are in place.
- Verify you are working on the correct component.
- Verify you have the correct rated Voltage absence tested and that it is calibrated. Prove the device and re-test and check for voltage at the point of work.
- If the voltage is still present, contact the AE.
What should be done if there is an issue discovered with the AWP during work on the WTG?
- Stop work and inform the working party to stop work and remain in a specified area that is safe.
- Contact the Authorising Engineer.
You have forgot the voltage absence tester, but you have a multimeter, can this be used instead?
No
What would you do in the scenario where work is being carried out in the hub and your supervisor has made you aware of lightening in the field?
- Stop all work immediately.
- Ensure the working area is in a suitable state to leave.
- Remove the team and head back to the vessel if possible, otherwise head to the designated safe zone within the tower.
- Once a green light is given by the supervisor monitoring the weather conditions, only then is it safe to return to work.
Explain how to enact the surrender process.
If surrendering the AWP…
- In section 6.0 of the AWP (Surrender Section), complete the date and time of the surrender along with the name and signature of the AT surrendering.
- In the “Comments” section, give detail of the step within the AWP that work has been stopped, also information about the task and at which point work has been stopped.
- All personal locks will be removed from the lockbox, leaving only the green control lock applied.
- The OC shall be made aware of the surrendered AWP before transfer back to the vessel.
- The lockbox/key for control lock and AWP should be handed over to the oncoming AT directly. If this is not possible, the lockbox/key and AWP should be handed into safe custody - this is a safe and secure location where the lockbox/key and AWP to only be obtained by an authorised person.
- The next AT will receive the lockbox/key and AWP from safe custody and once on the WTG will take control of the surrendered AWP from the OC.
- The AT will name, date and time and sign in the relevant boxes within the “surrender” section on the AWP.
- The AT will then verify that all isolations are in place and secure. He/she will countersign each isolation point within the AWP, prove dead and also prove dead at the point of work before setting his/her working party to work.
